By enrolling in the Evoke VIP Membership Program, you agree to the above terms and conditions.
Evoke Member Policy
• All memberships (Gold, Diamond, Platinum) require a 6-month minimum commitment.
• Membership fees are charged monthly to the credit card on file.
• Memberships automatically renew on a month-to-month basis after the initial 6-month term unless canceled with a 30-day written notice.
• 100% of your monthly membership fee is banked into your personal account and may be applied toward eligible treatments and/or skincare product purchases.
• Banked funds must be used within 12 months of deposit. Unused funds expire after 12 months.
• Banked funds are non-transferable and have no cash value.
• Gold Members: 10% off all treatments and skincare products.
• Diamond Members: 15% off all treatments and skincare products.
• Platinum Members: 20% off all treatments and skincare products.
• Membership discounts cannot be combined with other promotions, packages, or special event pricing unless otherwise specified.
• Early termination before completing the 6-month minimum term will result in a $199 early cancellation fee (equal to one month of Gold membership).
• A 30-day written notice is required to cancel or downgrade any membership after the initial term.
• Complimentary birthday treatment, Skincare or discounts must be redeemed within 30 days of your birthday.
• Must have an active Platinum membership at the time of redemption.
• One-time freeze per membership term allowed for up to 60 days (2 months), in case of medical needs, extended travel, or emergencies. Requests must be made in writing and approved by management.
• Membership is non-transferable and non-shareable.
• Membership privileges and policies are subject to change with 30 days’ notice.
• Evoke Medical Aesthetics reserves the right to modify, cancel, or restrict the membership program at its sole discretion with advance notice.
